Initial Appointment
Please arrive 10 minutes prior to your appointment to fill in a registration form and please bring with you:
• Your referral letter
• Medicare Card, DVA card, Pension Card and Private Insurance Details (if applicable)
• List of current medications
• Any blood results, x-rays or results from relevant investigations
Cancelling or Rescheduling Appointments
Should you need to cancel / reschedule an appointment, it would be appreciated if you do this as early as possible to ensure availability when re-scheduling and also to allow the appointment time to be given to another patient.

Paid underground parking is available at the RPAH medical centre. Enter off Carillon Avenue. Metered street parking is also available.
